Sometimes the themes of the claim and the reference are quite similar so it is hard to spot the mismatch. Sometimes the reference will have some relevant keywords in its discussion, but the main paper is actually not even investigating the claim. It might look OK on the surface but as soon as you dig into the meaning you see that something's wrong..
